When Am I Eligible for Insurance?
You are considered an eligible associate if your standard work week is at least 30 hours per week to qualify
for medical benefits and 32 hours a week to meet qualifications for all other FirstBank benefits. As a new
associate you will be eligible for coverage on the first of the month following the date of hire. However, you
must enroll in benefits within 30 days of your date of hire.

I Am Locked Out of ADP. Can You Reset Me?
HR cannot unlock your account. Associates can wait between 15 – 30 minutes and try again or they
can click forgot password. However, your account should automatically unlock within the 15 – 30
minute window and allow you to log back in.
When Do We Get Paid?
Every other week on Friday.
Can I Cancel a Time Off Request Once It Has Been Approved?
Yes, go to the request and click on cancel request. If it has processed through payroll, it will be
updated on the next pay period.

Eligible the first of the month following hire date.
If enrolled in medical, MD Live/Telehealth will be available at no cost to you with the exception of
the HDHP plan. Due to IRS regulations, MD Live visits will no longer be $0 cost under the HDHP
plan. Your cost per visit will vary, and the amount you pay will be applied to your deductible.
401K Election

Eligible the first of the month following 90 days of employment.
If no action is taken you will automatically be enrolled and FirstBank will deduct 3% of your pay
each pay period to fund your 401(k) account.
Medical and Pharmacy
FirstBank offers three medical plans through Cigna. Refer to the Cigna summary of benefits (SBC)
located on the Vault for a more comprehensive review. To view participating providers, visit
www.myCigna.com.
