2026 BENEFITS
MDLIVE
As part of your medical coverage, you have access to MDLIVE, which offers virtual care 24/7, 365 days
a year at $0 cost. MDLIVE virtual visits are covered at 100% before deductible for all medical plans,
including both PPO options and HDHP plans.
Available services include:
• Urgent care
• Primary care
• Dermatology
• Behavioral health
Vision
Coverage is offered through VSP. Visit www.vsp.com to find in-network providers and access to
a variety of online tools and programs. The vision plan is available at no cost but is a stand-alone plan
and must be elected by the associate. Please make sure to elect this coverage in ADP!
HSA
When you enroll in FirstBank’s high deductible health plan (HDHP), a health savings account will also be
opened for you. An HSA works similar to a personal savings account, but the money can only be used for
qualified healthcare expenses. The best benefit is FirstBank will contribute annually $800 to associates in
Tier 1, $600 to associates in Tier 2, $400 to associates in Tier 3 and $200 to associates in Tier 4!
FSA
A flexible spending account (FSA) allows you to set aside pre-tax dollars from your paycheck to cover
qualified expenses that you would normally pay out of your pocket for with after-tax dollars. Our
flexible spending account is offered through WEX.
Dental Coverage
FirstBank offers dental coverage for you and your family through Delta Dental of Tennessee, with both a
base plan and an enhanced buy-up option available.
Life and Disability
Life and disability insurance is offered through Prudential. FirstBank automatically provides you a benefit of
1 times your total benefit compensation *(up to $500,000) at no cost to you, for life insurance and accidental
death and dismemberment insurance. You are also eligible to purchase voluntary life insurance for yourself,
spouse, and dependents. Voluntary AD&D can be purchased in addition to voluntary life insurance.
Voluntary Benefits
Accident, critical illness, and hospital indemnity insurance is offered through Prudential.
Health Advocate
Health Advocate offers a unique level of healthcare, insurance and well-being support to help you
reach your best health. Our experts will do the work to ensure that you get the right information and
assistance at the right time. Our services are completely confidential and available to you, your spouse,
dependents, parents, and parents-in-law at no cost.
