Non Borrower Authorization Form
By signing this form, the borrower
authorizes the third party listed on
the form to obtain any information
on their designated mortgage
account with Shellpoint Mortgage
Servicng. The authorization does not
expire until revoked by the borrower.

Automatic Payment Enrollment
Form (ACH)
Shellpoint is pleased to offert a free
automatic payment option. The
borrowers monthly payments can
be automactially drafted from their
checking or savings account either
monthly, bi-weekly or semi-monthly.

drafting will reamin in effect until

If the borrower is experiencing
a temporary or long-term hardship
and needs help, they must complete
the Borrower Assistance Form and
submit any required documentation to
be considered for available solutions.
